Treatment Protocol
How many infusions you actually need
You don't need a lifetime of ketamine treatments. Most patients start with a defined induction series, then transition to maintenance — with the goal of eventually spacing out or stopping entirely.
The standard protocol
Induction series: what most patients start with
| Week | Infusions | Notes |
|---|---|---|
| 1 | 2 | First infusion establishes baseline response |
| 2 | 2 | Most patients begin noticing shifts by week 2 |
| 3 | 2 | PHQ-9 or GAD-7 reassessment at visit 3 |
| 4 | 2 | Full response assessment after 6th infusion |
6 infusions over ~4 weeks · response assessed at each visit
Why a series, not a single dose
The science behind the protocol
Neuroplasticity build-up
A single ketamine infusion triggers a glutamate surge that promotes the growth of new neural connections. But this growth is temporary without reinforcement — repeated infusions sustain the neuroplastic state.
Cumulative antidepressant effect
Research consistently shows that while a single infusion can produce 24-72 hours of relief, a series of infusions produces benefit that lasts days to weeks per infusion — and builds over time with repeated treatments.
Responders vs. non-responders
About 50-70% of patients respond to ketamine. Most responders see improvement by infusion 3-4. If you've seen no response by the 6th infusion, we reassess whether ketamine is the right approach for you.
Depression vs. pain protocols
Different conditions, different protocols
Depression, anxiety & PTSD
Lower dose: 0.5 mg/kg over ~45 minutes · 6 infusions over 4 weeks · private recovery in a comfortable room · most patients report a calm, dreamlike state
Chronic pain (CRPS, neuropathy, migraines)
Higher dose: 1.0-2.0 mg/kg over 3-4 hours · more frequent initial infusions (3x/week) · requires pretreatment with benzodiazepines to manage specific symptoms · longer recovery time
Common questions
Frequently asked questions
How many ketamine infusions will I need?
Most patients begin with a 6-infusion induction series over 4 weeks (roughly twice per week). After that, maintenance infusions are scheduled based on your response — typically monthly, quarterly, or as needed. About 50-70% of patients notice meaningful improvement by the 6th infusion.
Why do I need a series of infusions instead of just one?
While a single ketamine infusion can produce rapid relief lasting 24-72 hours, research consistently shows that a series of infusions produces longer-lasting, more durable benefit. Ketamine promotes the growth of new neural connections — a process that requires repeated dosing to sustain neuroplasticity over time.
What's the difference between depression and chronic pain protocols?
Depression/anxiety/PTSD protocols typically use a lower dose (0.5 mg/kg) administered over ~45 minutes. Chronic pain conditions like CRPS or neuropathy use higher doses (up to 1.5-2.0 mg/kg) administered over 3-4 hours. Pain protocols require longer sessions and often more frequent initial infusions (3x per week).
How will I know when to stop maintenance infusions?
There's no fixed rule. We use validated scales (PHQ-9, GAD-7) at each visit to track your response. When your scores stabilize in a healthy range for several months and you feel your quality of life has genuinely improved, we typically begin spacing out infusions. The goal is to get you to the point where you no longer need treatment — not to keep you on indefinitely.
Do I need to take time off work for each infusion?
Plan to take the rest of the day off after your first few infusions — you can usually work the morning after your infusion if your appointment is in the afternoon. As your body adjusts to the treatment, many patients return to work the same day with a driver.
Can I choose a different schedule than 6 infusions over 4 weeks?
We individualize every protocol. Some patients start with 3x/week for 2 weeks (6 infusions), others need 2x/week for 5 weeks. Your provider will recommend the schedule most likely to work based on your condition, prior response to treatment, and clinical judgment.
